PROGRAM DESCRIPTION
Community Case Management is a community-based program providing advocacy in obtaining services needed to improve the quality of life for consumers 18 and older who are diagnosed with a developmental disability and live at home with a specified adult relative or are living independently. Case managers meet with consumers and their families to provide coordination and delivery of supportive and other services by serving as role models, family mediators, advocates, mentors and resource coordinators through the development of person centered plans to empower each member to stay safe, healthy and an active member of their communities.
