PROGRAM DESCRIPTION
The St. Albans Community Alternatives Program (CAP), located in St. Albans, Vermont, is an intensive Family Wrap Around Service. CAP offers intensive clinical services to families to maintain the child in their current home and community.
St. Albans CAP offers three service components:First, Clinical Wrap Around Services to children at risk of residential placement. These supports range from 24-hour crisis services, case management, community integration, diagnosis and assessment, individual plans of care as well as therapeutic home placement. This is the largest component of CAP; working with children in biological, adoptive, and foster homes to develop and maintain safe environments to grow.
Second, CAP offers juvenile justice services that include Street Checking and Competency classes. These services offer individualized support to children and their families to promote successful completion of probation requirements and lower recidivism rates in the community.
Third, CAP offers supports to post adoptive families to maintain those adoptive homes. This is done through case management, crisis support and respite services.
